Some achievement on the VST front:
- New location: CLAM/examples/PluginExamples/VstPluginExample
- Main process related bugs have been fixed: control mapping and using the
proper process function for vst 2.4).
- VST plugins use embedded networks like ladspa plugins do.
- All hardcoded metadata turned into construction parameters.
- User and Library code has been splitted in several files
- Qt code has been integrated although it still needs work

I concentrated on cleaning up the code a user should write to get a plugin
which now is a single file: CLAMVstPlugin.cxx
Other c++ files in the folder are candidates to eventually become part of the
library (as happended with ladspa) but they a lot of cleanup.

It just works on Windows or crosscompiling from Linux with mingw.
