Uau, it seems an OpenGl driver problem in your machine. We had some problems 
with old nvidia driver on the windows machine. We solved that by checking our 
graphics card vendor for an updated one. (We found that reading some people 
reporting the same problem at the Qt mailing list). If after that you still 
have problems, let us know

There is a CLAM application Voice2MIDI which does that. You can find an 
install binary on the web (old windows binaries).
