[Clam-devel] multiple ports

Hi,

Again, the patch for multiple ports, but with the ladspa and portaudio
backend changed too.

The portaudio backend on linux needs attention as it is/was not 100% in
working order, I will have a look.

The patch was done from clam/CLAM so everything is in there.

The portcount starts at 1, for ease of use in the jackconnection viewer.
